This form is required to participate in the FHS Marching Band. Please fill out, sign, and return and/or email to Mr. Berz (dberz@franklinboe.org) or Mr. Diaz (ediaz@franklinboe.org). Health Forms and payment may also be handed in to Mr. Fairbanks or Mr. Daly.

In a team activity such as band, it is important for you to understand that your timely attendance is required at every rehearsal and performance. Marching Band students receive honors credit for their participation in rehearsals and performances; your attendance is a major component of your band class grade.
Please observe the following guidelines concerning attendance.
1) All Band and Color Guard members are responsible for every rehearsal and performance. The Fall Play, work commitments, non-emergency medical appointments, school work/test prep/test make-ups, school clubs, collegial visits, etc. will not be considered legitimate excuses. Please schedule these on days without rehearsals or performances. When you are absent from, or tardy to a rehearsal or performance, you must provide a written (hard copy or e-mail) parent/teacher’s note one (1) day prior to rehearsal or that performance as verification. Please understand that the note does not excuse you with regard to the attendance policy, or place your absence outside of our grading policy. It only confirms your whereabouts during the rehearsal or performance for which it is submitted.
2) The Band Directors reserve the right to determine whether an excuse is acceptable or not.
3) More than one (1) excused absence/tardiness during a week will be considered excessive. Any unexcused/absence/tardiness will be considered excessive.
4) Any unexcused absence from a Competition, Football Game, or any other Performance (including due to suspension from school) may result in a loss of your participation in the marching band program. (See #2 of Attendance Policy).
5) Unless excused by the Band Directors, all members must attend all performances in uniform,
whether they are marching on the field or not.
6) Continued excessive absence and/or tardiness to rehearsal/competitions (5 or more) will result in the
loss of your position in the field show and the termination of your participation in the marching band program.
7) Any behavior deemed disrespectful towards students, staff, or directors will have assigned consequences as follows:
● 1st offense- Phone call home and signed documentation from student regarding their behavior.
● 2nd offense- Suspension from program for 1 week (including rehearsals, game, and competition).
● 3rd offense- Removal from band program for rest of season. Students may join again the
following year if applicable.
8) Participation in the Marching Band requires your enrollment in one of the three band classes (Freshman Band, Symphonic Band or Wind Ensemble). Students weekly attendance will be reflected as part of their weekly Marching Band grade. Students will also receive a grade for our annual Memorial Day Parade. Students who are not enrolled in band class will not be eligible to participate in Marching Band. This does not apply to the Color Guard.
